Together We Can Make a Difference

 

Our Case Managers work with clients and their families in the home, school, and community to develop strength based individualized service plans designed to meet their physical, safety, and social-emotional needs.

We serve as advocates for the consumer and comfort them in knowing that the most fitting level of services is being made available.

 

Our Case Managers are certified through Florida's Agency for Health Care Administration (AHCA). All are experienced, and have advanced degrees in areas relevant to the services provided by the corporation.

Targeted Case Management services are long or short-term but transitional, focusing on the consumer's passage toward independence.

The Program is funded entirely by Medicaid.

Medicaid Eligibility Requirement:

To receive Medicaid or be Medicaid eligible with a ICD9 Mental Health Diagnosis 18 or over and have been diagnosed with a mental health diagnosis or under 18, receiving Medicaid or Medicaid eligible with a mental health diagnosis.
